Produced by the U.S. Department of Health, Education, and Welfare, U.S. Public Health Service, Bureau of State Services, Communicable Disease Center (CDC), Poliomyelitis Surveillance Unit (PSU).

Including: Three or more Cutter lab associated polio cases, one of which proved fatal, as well as a possible association between a vaccine distributed by Wyeth Labs and a case of polio involving limb paralysis which progressed to quadriplegia.
